Survey on the Mammographic Reporting : Questions and Answers about ACR BI-RADS Application

Abstract
Purpose: The purpose of this survey was to assess the kind of difficulties in wording the mammographic report and find out the solutions for them.

Materials and Methods: A questionnaire sheet consisting of 19 questions was created by collecting the kind of difficulties in the application of American College of Radiology Breast Imaging and Data System (ACR BI-RADS) in mammographic interpretation from 11 breast radiologists of 8 hospitals of the same university.The questionnaire was consisted of questions regarding dense breast, new terminologies introduced in the fourth edition of ACR BI-RADS and medical audit, etc. Two to seven examples were given for each question and only one answer was allowed to be selected. A total of 13 radiologists from the 8 hospitals working as either breast radiologist or in health care center were invited to fill out the survey via e-mail. The answers were analyzed and reviewed.

Results: The overall response rate to the survey was 100%(13/13). The consensus on answering was 10.5% (2/19) and 2 or more answers were selected for most of the questions(89.4%). In 15.7%(3/19), 4 or 5 answers were selected. This shows a considerate difference between the radiologists in assessment categorization and recommendation. Assessment category and recommendation for dense breast and global asymmetry and percentage of dense breast on screening mammography showed considerate difference.

Conclusion: Through our questionnaire, difference in opinion between the breast radiologists on interpretational standard and recommendation in application of ACR BI-RADS was found. If a consensus in mammographic BI-RADS application could be reached by reviewing the literature and we could establish a consistent interpretational standard, more effective breast examination and precise medical audit are thought to be achieved.
